When you first start up the game, if you're horrified by the cursor being a disembodied hand that periodically spurts blood onto the screen, then this isn't the game for you.

Graphics
Fairly standard for a game of it's time runing in DOS. Just enough polygons to make it look like a real city, but not so many that your computer's strained. The textures seem a little lacking in color depth, though. Resolution on them's a big issue as well. In the sequal, the pedestrians are polygonal and you can knock off arms and heads and such. Not here, they're flat sprites, and one touch and they're dead. You can increase the draw distance and such, but it's still not enough to see what's coming up on you after you reach speeds in excess of 150 MPH.

Sound
Never really noticed any background music, probably because I'm not using te CD version. Don't really miss it though, the wrenching sound of metal on metal mixed with blood spurts and screams creates it's own music. They use voice samples for everything, but a far too limited number. After rolling over four times and hearing the same "oh shit!" each time, it gets annoying. Thank god they have screams for males and females you run over. Once again, after about half an hour you've heard them all.

Gameplay
Oh yes, the fun part. Driving around and shtuff. Three ways to win a race, finish all the laps in first place. Probably the easiest and most boring. Kill all the opposing cars. Medium difficulty, and not boring at all. Kill every single person in the city. Hardest, gets boring after a while. Some of the cars are hard to kill because you can go up against dump trucks and the like, very heavy and hard to slam into a wall enough. There's one weapon against pedestrians, the electro-bastard ray. None against other cars. And the lack of any traffic makes it boring as well. There's parked cars and cops that chase you, but the streets are pretty much deserted.

Replayability
When you go up enough places in rank starting from 99th, you can enter new races. There's lots of races, like 30 or so. The thing is they're all just different courses around the same freaking five places. After you kill enough cars and earn enough money (credits) you can start to buy off cars that you kill and use them, so it's a bit more added fun. Each car has good points, like a slicing metel thing on the front for people, or big wheels for egtting around, but some aren't so fast, or are to low to the ground to even get up on a sidewalk. I'd recommend just trying to have as much fun with this game as possible instead of trying to be the best racer.
